What to Bring

You need to bring the following documents every time you come to the office. 

• • • Insurance Card

    • List of all your current medications, including over the counter supplements and vitamins

    • Any legal form regarding legal guardianship of a minor or disabled patient

What to Expect

After you check in, the receptionist will ask you for your insurance card and any relevant legal documents. During your first visit only, you will be asked to fill out a number of forms regarding your demographic and medical information. You will be given these forms to fill out in the waiting room. However, if you would rather fill out these forms in advance, you may download them, print them, and bring them with you to the office.
